Job Title: Case Manager
Position Number:
Work Days/Times: Monday-Friday; 7:30am to 4:00pm
Hours per Week: 40
Location: Palatine, IL
Description: The Case Manager is responsible for the coordination of service delivery to meet the needs of the adult individuals with a developmental disability or autism that we serve. The Case Manager is the liaison between the family or other service providers and conducts all staffings, and meetings in which personal goals for individual's served are established for the upcoming year.

  • Conduct pre-staffing meetings with individuals/guardians, in regard to accomplishments, program delivery and determination of who will be at the next staffing and when the next staffing will be held. A written record of these meetings will be provided and maintained in the individual's case records. Monthly progress notes are kept on individual's accomplishments towards established goals.
  • Ensure individuals have supports and are linked with resources such as: The Department of Rehabilitative Services, transportation providers, Social Security, Medicaid, etc.
  • Conduct monthly reviews of individual’s status in relation to their plan; provide needed adjustments or supports. Document in the individual's case record any status change or adjustments.
  • Recommend individual discharges as deemed appropriate and conduct the discharge process according to established guidelines.
  • Coordinate data collection for statistical reporting as required to document character of caseloads and to develop caseload analysis.
  • Assist persons served as needed with daily personal hygiene needs including use of the restroom, hand washing, dietary needs, accidents, seizures, etc. General Areas of Responsibility: Advisory and Resources
    • Serve as a resource about needs, progress, and problems related to the individual's served.
    Hours and Work Year
    • 7:30 a.m. - 4:00 p.m., Monday - Friday.
    • 40 hours per week.
    • 12 months per year.

    Requirements: Education/Training and Experience Requirements
    • Bachelor's Degree in the Human Service field.
    • li>Strong written and oral communications skills.
    • Possess a valid driver's license and a car available for work-related transportation needs.
    • Prior experience related to the field of Case Management desirable.
    Physical Requirements
    • Able to engage in physical activities required for the implementation of Non-Violent Crisis Prevention techniques (CPI), CPR and First Aid.


  • Job Title: Employment Specialist

    Location: Palatine, IL
    Description: Employment Specialist (American Reinvestment and Recovery Act grant funded position) The Employment Specialist is responsible for: •Providing support to individuals with developmental disabilities as assigned in the Community Employment Program. •Providing employment counseling, employment readiness assessment, job placement, job coaching and training to individuals with developmental disabilities as assigned. •Providing support to individuals with developmental disabilities in the areas of vocational services, independent living skills, communication, self-advocacy, and serves as an appropriate role model for individuals receiving services. •Providing support towards the transporting of designated individuals to and from Countryside Association, community employment sites, and community outings as requested. •Maintaining accurate, legible data on the performance of individuals with developmental disabilities including time sheets, documentation of performance objectives, job coaching information, and assessments of individuals served. •Providing efficient delivery of job coaching, and job placement services to individuals with developmental disabilities in the Community Employment program. •Providing support to the individuals with developmental disabilities within the Work Crews as needed. •Demonstrate excellent communication skills. •Demonstrate the ability to provide accurate billing information and necessary documentation. •Works with staff, and individuals with developmental disabilities served, potential and existing employers. Hours/Work Year •Employment Specialist position is a grant-based position. •40 Hours Per Week - 12 Months Per Year •Some evenings and weekends may be required. •The Employment Specialist is required to work outside the normal (7:30am to 4:00pm) Countryside Association working hours. •The Employment Specialist working hours are determined by the needs of the individuals with developmental disabilities served and/or the community employer.
    Requirements: Education, Training, and Experience Requirements •High school degree or equivalent. •Valid driver’s license and insurance card; driving record that will allow employee to be covered by agency insurance for the purpose of transporting individuals utilizing agency vehicles. •It is expected that the Employment Specialist may transport individuals in their own vehicle for employment-related activities. •Previous experience with adults with disabilities. •Demonstrate good written and oral communication skills. •Demonstrate ability to work independently and make appropriate, safe decisions.     Job Title: Respite Worker

    Work Days/Times: Hours are determined by the Respite client
    Hours per Week: 12 Hours per month
    Location: Lake County
    Description: At Countryside Association, we are dedicated to providing in-home respite care to children and adults with a developmental disability and/or autism. -Respite Workers provide non-medical in-home care and assistance for children and adults with a developmental disability and/or autism. -Countryside Association Respite Workers provide a helping hand, companionship, conversation, recreational activities, and assist Respite Clients obtain personal goals. -Countryside Association Respite Workers are compassionate and caring individuals that are reliable and dependable caregivers that enjoy providing care to adults and children with developmental disabilities -Respite Workers are placed in friendly and caring home environments of Respite Families. Respite Workers develop and maintain a positive, cooperative, and professional role with the Respite Client and their family. -Hours are determined by the Respite client's needs, funding, and worker availability. -Respite work is performed during the day, evening or weekend hours.     Job Title: Training Specialist - Full-Time

    Work Days/Times: Monday-Friday; 7:30am-4:00pm
    Hours per Week: 40
    Location: Waukegan, IL
    Description: The Training Specialist is the individual within the facility who provides job training and acts in a supervisory role with adults with a development disability, autism, or down syndrome in a workshop setting assisting individuals teaching job skills, and on-the-job training for manufacturing type piece-work. The Training Specialists provides training to adults with a disability making sure individuals participate in community outings, and socialization and life skills activities. Training Specialists help individuals reach goals with learning independent living skills, such as, money management, hygiene, cooking classes, or fitness as defined in the individual`s careplan. The Training Specialist maintains accurate, legible data on the performance of persons served including time sheets, performance on objectives, and assessments.
    Requirements: EDUCATION: High school degree or equivalent. Demonstrate good written and oral communication skills. Previous experience with individuals with disabilities desired. HOURS: 7:30AM-4:00PM - MONDAY - FRIDAY/40 HOURS WEEK. PHYSICAL REQUIREMENTS: Able to lift 40 lbs, using appropriate lifting techniques.     Job Title: Training Specialist/Part-Time

    Work Days/Times: 5 days a week; 9:00am - 3:00pm
    Hours per Week: 30
    Location: Waukegan, IL
    Description: The Training Specialist is the individual within the facility who provides direct training delivery and supervision to the adults that we serve with a developmental disability. Work with the adults in a group providing supervision and guidance in a workshop setting supervising the individuals completing their work, providing quality assurance to the finished work, and training the adults in job skills.

  • Assist persons served as needed with daily personal needs including use of the restroom, hand washing, dietary needs, accidents, seizures, etc.
  • Complete accurate paperwork on the finished work of the adults served including time sheets, performance on objectives, assessments, incident reports, accident - illness - seizure documentation, etc. Communicate data verbally and/or in written form in a timely manner as required by procedures.
  • Provide constant supervision for the adults served, keeping track of their whereabouts at all times.
    Requirements:
    • High school degree or equivalent.
    • Demonstrate good written and oral communication skills.
    • Previous exposure or experience with adults with disabilities desired.
    • Able to lift 40 lbs, using appropriate lifting techniques.
    • Able to engage in physical activities required for implementation of approved CPI,CPR and First Aid techniques.
